Immunizations

Florida Schools require children to have certain immunizations. These immunizations are free of charge for ages birth through 18.

The immunization clinic will issue the blue 680 form required for entrance in the public school system. Immunizations required may include: Hepatitis B; Diphtheria, Tetanus, Pertussis (DTaP); Haemobilus influenzae Type B; Polio; Measles, Mumps, Rubella (MMR); Varicella; Pneumococcal. Adult (3 injections required)
